Policy about filling out school/camp forms |

We are happy to complete your child’s forms for school,
camp, and daycare, and other activities.
As of May 1, 2007, there will be a $5 administrative fee for completing
any form not presented at the well care visit.
All forms that are presented at the child’s well care visit will be
completed for free. We will do our best
to complete them at the time of the visit.
All other forms will be subject to a $5 fee per form. Please provide a self addressed stamped
envelope along with the fee and the forms will be returned in five business
days. Please remember to write your
child’s name on each form prior to submitting it to us. There will be no charge for a printout of
your child’s immunizations at any time.
