Policy about filling out school/camp forms |

We are happy to complete your child�s forms for school,
camp, and daycare, and other activities.�
As of May 1, 2007, there will be a $5 administrative fee for completing
any form not presented at the well care visit.�
All forms that are presented at the child�s well care visit will be
completed for free.� We will do our best
to complete them at the time of the visit.
All other forms will be subject to a $5 fee per form.� Please provide a self addressed stamped
envelope along with the fee and the forms will be returned in five business
days.� Please remember to write your
child�s name on each form prior to submitting it to us.� There will be no charge for a printout of
your child�s immunizations at any time.
